Parent Job Shadow Approval

 Learning is easier for Academy students who, through observation, gain an understanding of how business works. Academy mentors offer students the opportunity to gain an understanding of a typical work day.

Students will shadow their mentors for part/all day. School personnel will not be present when the student is at the site. Students are responsible for providing their own transportation and meals.

I give permission for my son/daughter to be released from school to visit a job site and watch the above named mentor at work. I agree with the gravel and supervision arrangements states above. I understand that my son/daughter will be in a real work setting and my be exposed to any job hazards associated with this business. I agree to hold ________________ School District harmless from any and all liability or claims which may arise out of/in connection with my child’s participation in this program.
